Title: Detection of mammaglobin mRNA in the blood of breast cancer egyptian female patients and its relation to established prognostic parameters

Biography:

Dr. Amal Fawzy: assistant professor of clinical pathology -National Cancer Institute – Cairo University since 2012.One of the biggest oncology centers in the Middle East. More than 15 publications in the oncology field especially solid tumors (breast, ovaries and liver).

 

Abstract:

Abstract:
Mamma-globin, also known as secreto-globin family 2A member 2 (SCGB2A2), is a member of the superfamily of secreto-globins. Its expression is highly specific for mammary tissue and has been shown to be overexpressed in breast tumor tissue, indicating that mamma-globin might confer a growth advantage to mamma-globin-expressing tumor cells. This study included 46 breast cancer patients, 20 patients diagnosed to have benign breast tumors, and 10 apparently healthy volunteering females as normal controls. Mamma-globin expression was detected in the peripheral blood of both patients and controls using real-time RT- PCR. Mamma-globin was detected in 26% of peripheral blood of breast cancer patients studied but not in any of the benign or healthy individuals. It showed statistically significant relations with the positivity of HER2neu expression, the presence of distant metastasis and with CEA (p- value =0.005, 0.013, and 0.001 respectively). On the other hand, it was statistically non- significant for age, grade, stage, ER or PR positivity, size of the tumor, and Lymph node involvement. It showed 26% sensitivity and 100% specificity. Our results suggest that mamma-globin is a specific molecular marker for detection of occult mammary carcinoma cells in the peripheral blood of patients with operable breast cancer. It might be of value as a predictor of subsequent metastasis.

Title: Near infra-red, non-invasive serum creatinine determination and monitoring

Biography:

Z Barnea Burbea is working as a nephrologist in Wolfson Medical Center, Nephrology, Holon, & TA University, Israel.

Abstract:

The aim of the study to develop an online, non-Invasive serum creatinine measurement and monitoring eGFR in acute kidney injury (AKI) & Chronic Kidney Disease (CKD) set up. A method that can be applied in various situations, where AKI can develop e.g. during cardiac catheterization, sepsis, hemodynamic insult and other situations. The method is based on near infrared (NIR) spectrometry. An oximeter like manner approach was applied for serum creatinine. It is based on illuminating a light beam on the skin where a detector collects the reflected beam. The back light withholds information on the target molecule. The obtained data is processed through an algorithm of prediction/calculating resulting in a real-time eGFR determination.  60 blood samples from normal individuals, CKD & hemodialysis patients with serum creatinine of 0.8-12.4 mg/dl traditionally determined were NIR analysed in special tubes for simulating human fingers for comparison between the suggested method and the traditional one. Correlation of 0.99% were obtained (Figure) between the reference samples (traditionally determined) results and the calculated concentrations by using NIR analysis of the diffracted/reflected spectra. A reliable eGFR monitoring via an optic method can be applied in ICU, medical departments and any point of care CKD and AKI risk situations.

Abstract:

So far, medical knowledge, as benign and malignant diseases appearance causes, distinguishes cancero-genous substances, viruses and physical agents. In our researches, over 30 years, we have proven and demonstrated that physical fields, magnetic and variable magnetic, are benign and malignant diseases appearances causes. Cancero-genous substances and viruses, according to the official medical literature, are risk factors, and not causes. Our researches focus are exactly these physical fields, magnetic and variable magnetic (originating from electro-magnetic fields). Using the previous geophysical knowledge, geophysicist Nikola Trifunovic, graduated engineer, has, on microscopic apparatuses, recognised in diseased persons, similarity between changed cells (cancer's pearls) and serpentine rocks (magnetite's little eyes). This discovery, that magnetic and variable magnetic fields at one side, and changes on cells and microscopic view of serpentines, at another, are visually very similar, has suggested a team intensive research necessity in this field, a team consisting of both a medicine expert and electro-technics field expert. This team holds this workshop. For years, we have presented our researches at international congresses and published in international magazines. At the same time, we have studied and followed researches in medicine and other professional literatures. Natural Earth magnetic field with cosmic radiation is a life appearance and evolution on Earth fundament. At first, we measured all physical fields in people's dwelling places. Measurements were performed by objective geophysical methods with measurement instruments. Magnetic and variable magnetic fields measurements gave objective results. We used photonic magnetometer, punctuality of 100 nT, and compass "Brunton" punctuality of 1 degree in urban spaces. We concluded that variable and changing magnetic fields were the only ones that sum up with natural Earth magnetic fields, which we named anomalous magnetic fields. Anomalous magnetic fields appear in people's dwelling places due to all magnetics and paramagnetic and variable magnetic fields appear due to electro-magnetic fields. Anomalous magnetic fields are causes of benign and malignant diseases, where the exposition length is very important (in order that Para-oncogene mutates into oncogene). Various types of these fields are causes of many other diseases (diabetes mellitus, asthma, cardiovascular, mental, autoimmune, as well as many other diseases). In our works published in local and international scientific magazines, we clarify many diseases appearance and suggest preventive and therapeutic possibilities.

Nikola Trifunovic is a graduate engineer of geophysics, an honorary member of the Serbian Medical Society and a member of the Serbian Academy of Innovation Sciences. Investigated and analysed for over 25 years, the impact of the Earth's magnetic field (ZMP) on bio-systems and even on humans. He found that the unnatural ZMP were the triggers of the mentioned diseases.

Targeted cancer therapies are increasingly used in oncology care as single agents or in combination with other classes of oncology care. Like other treatment options, targeted therapies are associated with adverse events which may cause treatment adjustments and deterioration of quality of life. Targeted therapies may induce adverse event burden, with or without significant clinical evidence of tissue damage, while having far reaching consequences for patient adherence with care and therefore cancer treatment outcomes. To be able to complete treatment as planned and to maintain quality of life, adverse events need to be addressed according to the patient’s needs. Therefore, the patient’s view is a critical component of an integral approach to adverse events of targeted agents.

For the best treatment outcomes, you will need to solve the challenges that patients encounter when on targeted anticancer treatment. One of these challenges are the occurrence of adverse events. The level of adverse event expertise you personally need depends on the level of expertise the other team members are on and the clinical trials you are involved. In all cases, initiation of the most appropriate adverse event treatment is more likely to occur if the adverse events are addressed with the 6-step practical approach. Running an effective patient consultation  requests special habits. The institution of adverse event subtypes is one of those habits. The ASK method is another success habit. Title: Aging and death occur as a result of cells’ magnetic properties weakening and blood vessels’ volume decrease

Abstract:

All that exists in the Earth magnetic field (EMF) receives paramagnetic and ferromagnetic magnetization, that is why tissue cells and organs have magnetic properties (MP). EMF magnetizes all the constructing cells' parts and these MP, mostly, enable metabolism in the bioworld. Cells division, appearance and treatment of cancer, aging and death are subject of many studies, but full explanation, until now, has not been given. Sequences and nucleotides in DNA and RNA construction match as "key and lock" only according to a magnetic code. Crossing over  enables polymorphism and contributes to evolution enabled by EMF. Factor - promotor of mother cell division into two baby cells is EMF. Intermolecular magnetic forces (Mf) in cell are variable because cell's magnetisation center is in nucleus intensity depending upon number of divisions. With every division telomeres lose 100-200 nucleotides, reducing Mf in nuclei and metabolism in cells. Immune system weakens, too, because it functions due to MP. It is clear that immune systems functions the best in natural EMF.Show that EMF are factors inducing cells division. Explain that metabolism in cells happens, mostly, due to nucleus, orgenelles and substances MP. Clarify aging causes and mechanisms on cells, organs and organism level; how immune system functions in EMF and how, with aging, organism's deffence against intruders weakens; how immune system functions the best. Literature on cells division and functioning study. Aging and cells deaths reasons study and how immune system functions in EMF. Metabolism in bioworld and immune system functioning, mainly, enabls EMF. All aging manifestations are clearly understood by cells MP reduction and atherosclerosis. Natural EMF is the cells' division inducing factor. Nucleus, organelles and substances MP enable cells metabolism. Cells EMF and MP have decisive contribution to bioworld evolution processes. In division, cell's nucleus' Mf weaken, meatbolism, too, resulting in less water entering cell and toxines accumulation. Some organs' functions get reduced, especially those organs secreting hormones. By aging there also happen calcium salt in cartilage (in and around hondrocites, too) deposition, than, immune system functions more weakly, and all this is organism aging manifestation. Parallel with cells MP reduction go atherosclerosis processes, blood vessels elasticity reduction.

Title: Malignant diseases appearance in anomalus magnetic fields

Abstract:

Natural Earth magnetic field (EMF) is mother cell's division into two baby cells promotor. Cells that divide endlessly are cancer cells distroying organs and organism. Prove that malignant diseases appear in anomalous magnetic fields (AMF). Clarify unclear topics from medicine literature concerning etiopathogenesis. Magnetic fields measuring by protonic magnetometer. Particularly, measuring in patients' beds because of exposition duration. Measurment points per square net 10x10 cms. If measurements discover AMF, the patients are moved into natural EMF and their health conditions followed. The research lasted for 30 years with both sexes and all ages, totalling to 2000 cases. The diseased were in AMF, correlating with diseased organ's location. Shown are AMF surfaces in beds where affected organs were located. On the rest of the beds' surfaces were natural EMF. In a group of 30 patients with breast malignancy with metastastasis and after mamectomy, only 3 survived, thanks to locating in natural EMF.

1. Based upon results, magnetic fields in patients' beds measurements correlation between AMF and the diseased organ's found. 2. Many years following health condition during  patients' stay in natural EMF, health condition improvement and no recidives found. 3. Uncler topics from medical literature, i.e. etiopathogenesis, clerified based upon knowing AMF and natural EMF effects. All presented gives conclusion that AMF located in beds are the only cause of malignant diseases appearance. The discovered cancer cause imposes the obligation to improve treatment and prevention, which is in spending the night and day rest in natural Earth magnetic field.

Title:  A kidney health study on mornington peninsula population

Biography:

Alinda Chiu is an Australian based consultant nephrologist and physician working in the private sector at Peninsula Private Hospital (Frankston), Beleura Private Hospital and The Bays Hospital (both in Mornington) and the public sector at Frankston Public Hospital. She studied and trained in the UK becoming a Member of the Royal College of Physicians UK in 2000 and becoming a Fellow of Royal Australian College of Physicians 2010. She, together with her dedicated nurses and staff, engages in charitable works by running World Kidney Event Days in 2015, 2016 and 2017. The aim of these events is to raise awareness of kidney health to the public. Information gathered during these events is used for research into the prevention or delay onset of Chronic Kidney Disease. In addition to these events she organised an international standard educational seminar for nurses on 1st March 2017 at Peninsula Private Hospital. Other charitable activities include the sponsorship of children in Sri Lanka and Malaysia.

Abstract:

A kidney health check was organised on World Kidney Day in 2015 and 2016 at Beleura Hospital, a private hospital in Mornington, Melbourne. The kidney day was conducted over a day in each year. The participants include health workers, hospital staff, mobile patients and members of the public. All participants consented prior to being involved in the study. The 8 hour event was run by doctors, nurses and volunteers. Measurements of blood pressure, height and weight were taken from each participant. A Q risk questionnaire was completed. The issues of being overweight and obesity were common in this population. 36% were overweight and 35% were obese (noted in 2015).  In 2016, 38% of the participants were overweight and 26% were obese. In the group of participants, a mere 8% in 2015 were smokers and only 6% of the participants in 2016 were smokers. The risk of developing chronic kidney disease over the next 5 years was higher in the diabetic group (noted in 2015). This was also evident in the 2016 study. The risk of needing to undergo dialysis or a kidney transplant in the next 5 years was noted to be marginally higher in the diabetic participants (noted in both years). The risk of having chronic kidney disease within the next 5 years was found to be higher in the participants with cardiovascular disease (noted in 2015 and 2016) Participants with hypertension were noted to have a higher risk of developing chronic kidney disease in the next 5 year (noted in 2015 and 2016).The risk of developing chronic kidney disease in the next 5 years was higher in the overweight and obese group as compared to normal weight group (noted in both 2015 and 2016). In the 2016 study, there was a steep increase in the risk of developing chronic kidney disease in 5 years in the obese group as compared to the overweight participants. In summary, the risk of developing chronic kidney disease in the next 5 years was high in the participants with diabetes mellitus, hypertension, a history of cardiovascular disease and those who were overweight or obese. A kidney health check was conducted in two private hospitals (Peninsula Private Hospital and Beleura Hospital) in 2017. 243 participants enrolled in this study.  The study population were mobile inpatients, members from the public and health workers (nurses, doctors, allied health workers and hospital staff). The majority was aged 65 to 75 years old (aged population).

Title: Investigation of mechanisms that are involved in the pathogenesis of diabetic angiopathy and nephropathy and development of indicators of scouting diagnosis of the above complications

Abstract:

To evaluate the role of extracellular matrix-associated (Glycosaminoglycans - GAGs, tissue growth factor - CTGF), angiogenic (VEGF) and inflammatory factors (MCP-1, CD40, IFN-γ) in the development of diabetic nephropathy in type 1 diabetes (T1DM). Plasma and urine samples from 30 T1DM patients and 20 healthy controls were used to measure the levels of CTGF, VEGF, MCP-1, CD40 and IFN-γ by ELISA. Plasma and urine GAGs were measured using a spectrophotometric method. Plasma levels of GAGs and CD40 and MCP-1 and also urine levels of GAGs and CTGF were significantly elevated in normoalbuminuric T1DM patients. A tendency to higher plasma VEGF levels was found in patients compared to controls. The urine/plasma GAGs ratio of T1DM patients was almost similar to that of healthy subjects, whereas the urine/plasma CTGF ratio was about three times greater in diabetic patients compared to healthy subjects. Increased GAGs and CTGF excretion are evident in T1DM normalbuminuric juveniles, possibly reflecting early renal injury signs, before the initiation of albuminuria.

Title: Employment status of patients with ESRD and the subsequent treatment modalities in the United States

Biography:

John D Sullivan is an expert in health care finance, law, and policy, as well as mergers and acquisitions and asset valuation. He has taught at Boston University’s Metropolitan College for over 25 years, and served as chair of the Department of Administrative Sciences since 2015. Prior to joining BU, he worked as a bond analyst specializing in high yield corporate debt and business development, overseeing the acquisitions and valuations of over 100 transactions totaling $1.5 billion for W.R. Grace. In 2008, He co-founded Reliant Renal Care with $50 million in private equity funding. He was granted a faculty position in the BU School of Law in 2012, teaching International Transaction and Merger Law for the LLM Program. From 2014 to 2016, he was a research fellow with the United States Department of State’s Office of Threat Counterterrorism and Office of Threat Finance Countermeasures. He has served on multiple boards of directors and consulted for numerous large health care companies across the United States.

End Stage Renal Disease impacts the lives of over 700,000 Americans and their families. Despite the cost of supporting such a chronic condition, the United States initiated a system whereby everyone would be covered that qualifies for social security benefits, regardless of age. However, over three decades, the dialysis via two large provider chains (one fully integrated) took over and led the patients ultimately to a choice of hemodialysis in the out-patient setting without a somewhat technological option of home hemodialysis or peritoneal dialysis. This may have occurred as a result of medical training although the system is structured more on a corporate basis. That said, patients seem to be driven towards outpatient hemodialysis over other options such as haemodialysis and peritoneal dialysis. The result has created an economic system that has shifted toward what is perceived as the lowest cost of care without consideration for other treatment modalities that promote a better quality of life for the patient as well as monetary benefits for the United States Government. From a purely economic standpoint, profitability for providers is in the out-patient clinics despite the initial investment of $1-2 million via fixed asset utilization and the variable cost of peritoneal dialysis as well as the supply costs needed for home hemodialysis. In other words, for these two treatment modalities, the margin, if any, is built into the treatment. Margins can be enhanced through the utilization of in-centre facilities. With the focus on this modality, there are economic impacts based on decisions that increase the overall expenditures to the entire system that can easily be avoided.

Abstract:

Cardiovascular disease and osteoporosis are major age-related causes of morbidity and premature death that occur among older adults. Emerging evidence suggests that calcium from bone is deposited in the arteries. This leads to calcification in valves and vessels, which is associated with a 3-4 fold higher risk of coronary heart disease. Calcium and vitamin D supplementation is frequently advised in older adults for the prevention of osteoporosis and to reduce the risk of fractures and falls. However, recent RCT’s showed that individuals taking calcium and vitamin D were at higher risk of cardiovascular disease. The widespread use of vitamin D and calcium supplements, and the fortification of vitamin D in food products, underscores the need for further investigation. Recent studies show that vitamin K reduces coronary calcification and cardiovascular disease. New evidence suggests that vitamin K combined 
with vitamin D can lower vascular calcification more than the effect of either alone. This presentation will give a brief overview of the current body of evidence of joint associations between vitamin D and vitamin K with hypertension and underlying mechanisms of cardiovascular disease in multiple cohort studies in chronic kidney disease patients. The combination of low vitamin D and K status is associated with increased blood pressure and might play a role in development of hypertension and cardiovascular disease risk. These promising results of the joint associations of vitamin D and vitamin K in relation to cardiovascular health stimulate further follow-up studies to better understand the clinical implications of this relationship to promote cardiovascular health.

Title:  The HIPPO signaling pathway in cervical carcinogenesis

Biography:

Research in Dr. Wang’s laboratory focuses on uncovering the cellular and molecular mechanisms underlying the development of cancers in female reproductive organs, aiming to facilitate the effective prevention, early diagnosis and better treatment of these cancers.

Abstract:

Although accumulating evidence indicates that human Papillomavirus (HPV) alone is insufficient to induce cervical cancer cervical cancer (CVC), the exact molecular mechanisms underlying CVC development are largely unknown. Our previous studies indicated that the Hippo pathway played a critical role in regulating the proliferation and malignant transformation of cultured cervical epithelial cells. However, whether the Hippo pathway also drives cervical carcinogenesis in vivo is unknown. Unique cellular models & transgenic mouse models were used to examine the potential role of the Hippo pathway in the initiation and progression of cervical cancer and explore molecular mechanism(s) by which the Hippo pathway drives the cervical carcinogenesis. Doxycycline-induced expression of YAP1 in the cervical epithelial cells led to neoplasia of cervical epithelium within two months in KRT14-YAPS127A mice. Most KRT14-YAPS127A mice developed invasive squamous cell carcinomas (SCC) after induction with doxycycline for six months. Metastasis of SCC induced urethral and outlet obstruction in some KRT14-YAPS127A mice. Around 50% KRT14-YAPS127A mice died of SCC within 16 months of doxycycline induction. Compared to KRT14-YAPS127A mice, KRT14-E6/E7-YAPS127A mice developed SCC faster (within four months of doxycycline induction). Cancer cells derived from KRT14-E6/E7-YAPS127A mice were more proliferative and invasive. Mechanistic studies showed that ectopic expression of YAPS127A in human cervical epithelial cells resulted in a significant increase in cell susceptibility to HPV infection, while inhibition of YAP1 activity reduced the susceptibility of cervical epithelial cells to HPV infection. Intriguingly, we found that activation of YAP1 induced up-regulation of the putative HPV cellular receptor and disrupted innate immunity in cervical epithelial cells. Multidimensional cancer genomic data showed that disruption of the Hippo signaling pathway is a common event in human CVC, validating the clinical relevance of our findings. Our research demonstrates that constitutive activation of YAP1 is sufficient to induce cervical CVC. Hyperactivation of YAP1 increases the susceptibility of cervical epithelial cells to HPV infection. HPV, in turn, synergistically promotes YAP1-induced initiation and progression of cervical cancer. The Hippo/YAP signaling pathway may represent a promising target for developing novel strategies to prevent and treat cervical cancer.

Abstract:

Background: Breast cancer (BC) is the most common cancer affecting women. DNA is normally released from an apoptotic source which generates small fragments of cell-free DNA, whereas cancer patients have cell-free circulating DNA that originated from necrosis, autophagy, or mitotic catastrophe, which produce large fragments.

Aim of work Differentiate the cell free DNA levels (cfDNA) and its integrity in breast cancer patients and control group composed of benign breast lesions and healthy persons. Methodology: cf-DNA levels were quantified by real-time PCR amplification in breast cancer

patients (n = 50), breast cancer lesions(n = 25) and healthy controls (n = 30)using two sets of ALU gene (product size of 115 bp and 247-bp) and its integrity was calculated as a ratio of qPCR results of 247 bp ALU over 115 bp ALU. Results: Highly significant levels of cf-DNA and its integrity in breast cancer patients compared to control groups. Twenty-eight (56%) patients with breast cancer had metastasis. ALU115 qpcr is superiorto the other markers in discriminating metastatic patients with a sensitivity of 96.4% and a specificity of 86.4% and (AUC= 0.981)Conclusion: ALU115 qpcr could be used as a valuable biomarker helping in identifying high risk patients, indicating early spread of tumor cells as a potential seed for future metastases.
