Title: Neonatal Diabetes Mellitus: An Update on Diagnosis and Management

Biography:

Dr. Mahmoud Metwaly Taha has a Master's Degree in paediatrics and neonatology awarded from Zagazig University, Zagazig, Egypt. Currently working as a senior neonatologist at Saudi German Hospital, Aseer, KSA.

Abstract:

Neonatal diabetes mellitus (also termed congenital diabetes, or diabetes of infancy) is highly likely to be due to an underlying monogenic defect when it occurs under 6 months of age. Early recognition and urgent genetic testing are important for predicting the clinical course and raising awareness of possible additional features, and in many cases, these are essential for guiding appropriate and cost-effective treatment. Additionally, early treatment of sulfonylurea-responsive types of neonatal diabetes may improve neurological outcomes. It is important to distinguish neonatal diabetes mellitus from other causes of hyperglycemia in the newborn. Other causes include infection, stress, inadequate pancreatic insulin production in the preterm infant, among others. Insulin-dependent hyperglycemia that persists longer than a week should raise suspicion for neonatal diabetes mellitus and prompt genetic testing.

Title: American Diabetes Association Standards of Care - Moving From Knowledge to Adoption

Biography:

Robert A. Gabbay, MD, Ph.D., FACP, is the Chief Scientific and Medical Officer for the American Diabetes Association (ADA), the global authority on diabetes. Dr. Gabbay leads the ADA’s efforts to drive discovery within the world of diabetes research, care, and prevention.

Abstract:

The 2022 Standards of Medical Care in Diabetes includes all of ADA's current clinical practice recommendations and is intended to provide clinicians, patients, researchers, payers, and others with the components of diabetes care, general treatment goals, and tools to evaluate the quality of care. The recommendations are based on an extensive review of the clinical diabetes literature, supplemented with input from ADA staff and the medical community at large. The Standards of Medical Care in Diabetes is updated annually, or more frequently online if new evidence or regulatory changes merit immediate incorporation and is published in Diabetes Care.  Four key themes emerge the 2022 Standards-new recommendations on screening for diabetes, individualizing care, management or comorbidities, and the glycemic assessment with technology.

Title: In vitro and in vivo anti-Cryptosporidium and anti-inflammatory effects of Aloe vera gel in dexamethasone immunosuppressed mice

Biography:

Abstract:

Cryptosporidiosis has been considered as a serious diarrheal disease, especially in immunodeficient patients, where they failed to clear the infection leading to several consequences of infection (i.e death). The role of cell mediated immunity in clearing the infection was demonstrated by the increased susceptibility of HIV/AIDS patients to infection. To date, no specific treatment has been proven for cryptosporidiosis in immunodeficient patients. The study aimed to evaluate the efficacy of Aloe vera gel for the treatment of cryptosporidiosis in immunocompetent and dexamethasone immunosuppressed mice in comparison to that of nitazoxanide. Mice were orally administrated with Aloe vera gel, in a daily dose of 250 mg/L in drinking water, for 14 consecutive days post infection. Parasitological, molecular and immunological measurements were recorded on the 7th, 14th, 21st and 32nd days post infection. Our in vitro results showed that 250 mg/L of prepared gel achieved the highest parasitic reduction. The body weights of Aloe vera treated mice on the 21st and 32nd day post infection, either in immunocompetent or immunosuppressed groups, were nearly the same as those of their corresponding control groups. Aloe vera gel succeeded in clearing cryptosporidiosis with a percent reduction of 100% in immuno- competent mice and 99.67% in immunosuppressed mice. The anti-inflammatory effect of Aloe vera reduced the levels of IFN-γ, IL-4, -6 and -17. The success of Aloe vera gel, in clearing cryptosporidiosis in immunosuppressed mice, was obvious either from the reduction of Cryptosporidium DNA or the oocysts in stool samples; and from the improvement of histopathological sections.

Abstract:

Diabetes, Global burden of 537 million adults with the continuous increase of its incidence and prevalence, and it is estimated that if the pattern of rising remains the same then after 25 years it will involve 783 million people. Sedentary lifestyle, urbanization, aging population, an increase in obesity ratio being the highest risk factor. Such an increase in prevalence becomes challenging for health care practitioners to manage every patient on an individualized basis. Currently, Diabetes care is facing many challenges, and to overcome these challenges, there was a need for increased communication between clinicians and patients, customization of care, and increased collaboration among them that become helpful in improving health outcomes and preventing diabetes complications Digital health is a convergence of health care and technology in which integration of collected data by using software and apps, the emergence of modern models of care, driven by the ubiquity of mobile devices, provide better information, offer low cost with best possible outcomes. Telemedicine is the “Exchange of medical information with the use of electric communication for improving patient’s health status" according to the American telemedicine association. It is an extended term of medical care remotely by the use of audiovisual communications. It includes the transmission of patient data and its remote interpretation for follow-up along with preventive intervention The fact cannot deny that patient-physician interaction plays an essential role in improving health outcomes and preventing long-term complications. Telemedicine proved its role in Diabetes Management and it promises to become a Novel 21st-century tool for better communication between clinicians and patients in a cost-effective manner.
